What to Bring

  • Photo ID (driver’s license or state ID)
  • Insurance card (front and back)
  • Referral or prescription from your doctor (if applicable)
  • Comfortable clothing appropriate for movement (shorts, t-shirt)

  • Your first visit will last approximately 60 minutes
  • A one-on-one evaluation with your Doctor of Physical Therapy
  • You’ll work with a consistent care team of no more than two therapists — no rotating through unfamiliar faces every visit

FAQ and Logistics

Do I need a referral to be seen?

In Michigan, you can see a physical therapist without a referral for up to 21 days. However, some insurance plans may require a physician referral for coverage. We recommend checking with your insurance provider or contacting our office.

What insurance do you accept?

We accept Medicare, Priority Health, Blue Cross Blue Shield, Aetna, Cigna, Cofinity, FirstHealth, Tri-West, Molina, Humana, ChampVA, Workers' Compensation, and Auto Insurance (PIP). Contact us to verify your specific coverage. Self-pay rates are also available for patients without in-network coverage.

What should I wear to my appointment?

Wear comfortable, loose-fitting clothing that allows for movement. Athletic shorts, t-shirts, and sneakers are ideal. We recommend clothing appropriate for access to your intended treatment area.

Is your facility wheelchair accessible?

Absolutely. Our entire facility is fully ADA-compliant and designed from the ground up to accommodate wheelchairs, walkers, and other mobility devices. Accessibility is at the core of everything we do.
